Hi Parents,

Our new insurance requires all participants to have a signed/stamped physical in 2025. We are no longer able to allow kids to play with ones from the previous year. We understand that your insurance only covers physicals once a year, so if your child is receiving their annual physical before July 31, 2025, then you should be all set. If your child is scheduled to get the annual physical after July 31, 2025, all we need is the physical waiver that is attached to be dated, signed and/or stamped by your physician anytime before July 31, 2025.

Once complete please return to your league/coach ASAP. Without this form signed/stamped and dated or a 2025 physical you child will not be able to participate in the 2025 season.
